Collect additional signatures in one submission

Collect witness, guardian or other signatures alongside one response.

Use separate signature fields when more than one person completes the same form submission. Examples include a parent and witness completing a consent form together, or two parties signing while reviewing the same set of answers.

Choose the right arrangement

Additional fields in one submission are useful when the people can complete their parts in the same session. A workflow is useful when a request must move to another person's inbox, wait for their action, or restrict their part of the response.

Decide this before adding fields. Two required signature fields do not, by themselves, create two invitations or a sequence of emailed tasks.

Add clearly named fields

  1. Add the first signature using the signature-field setup.
  2. Add another signature for each additional person who needs to sign this submission.
  3. Place each signature after the relevant acknowledgement or signer details.
  4. Give the fields distinct names, such as Parent signature and Witness signature.
  5. Check requiredness for every added field. Do not rely on the first field's default to configure the others.
Configure each person's signing task separately.

Check the complete submission

Preview the form from the same link you intend to share. Check the entire sequence of questions and signature fields. Make sure the accompanying wording explains each person's role and what they are acknowledging.

If an additional signer is genuinely optional, configure that field accordingly and check the missing-signature result. A required field should block completion when it is left empty. Distinguish this from a workflow's optional-recipient behavior, which depends on whether an email address is supplied for that response.

Inspect the sample response and any generated PDF so signatures can be matched to their labels. Attachments, notifications and Drive copies need their own settings; adding another field is not evidence that every signer will automatically receive a separate copy.

When to move to a workflow

Choose a workflow if a manager must sign after an employee, recipients are in different locations, or staff need a pending-task view. Configure signer roles, email addresses, signing order and the request channel there.

If a respondent reports that a second signature is missing, first confirm whether they opened the correct Formesign link and reached the relevant field. Then inspect requiredness and the response itself before resending anything. See request troubleshooting for a response that remains incomplete.
