Reports and exports
Choose useful response columns, filter requests and export the data your team needs.

Reports help you compare responses without opening each signed document. Use structured columns for tasks such as finding pending requests, reviewing a period's intake or listing equipment requests by department. Keep the signed record available when the document itself is needed.
Open and choose the report
Open the form's response/report controls and select the report view offered for that form. Some report surfaces use Google Sheets as the label for a tabular view inside the product; a view with that name is not automatically a connected Google spreadsheet.
Choose the relevant time period and columns. Include an identifier, date and status so staff can trace a row back to the correct response. Add only the question values needed for the task; a large export can make sensitive answers harder to control.
Review a useful example
For an equipment request, show employee name, equipment description, department, submitted date and status. Filter the pending work or sort by date. Before acting on a row, open its response to confirm the current workflow step and signature state.
A patient-intake report may need a much narrower set of columns. Do not include medical answers in a general completion-tracking view merely because they exist in the form.
Export or connect a Sheet
Depending on the report surface and plan, controls may offer Export, Export to Sheets or Link to Sheets, with CSV or PDF options in report menus. Use the control currently shown for the selected report.
An exported file is a point-in-time copy. A linked Sheet is a configured connection whose destination and sync behavior must be checked. Follow Google Forms and response Sheets when you need ongoing output.
Review sensitive-field choices
Reports can prompt about marked PHI fields. Review that choice instead of assuming every answer is included or excluded automatically. Check the destination's sharing permissions before exporting sensitive columns. A report's PHI selection does not establish how every other external connection handles the same data.
For the completed document, use signed PDFs; for privacy configuration, read PHI controls.
