Health & Safety inspections and incident reports
Collect inspection and incident details, route the next task, and retain usable signed records for safety-team follow-up.

Use Formesign to collect an inspection or incident report with the supporting files and signatures your process needs. The useful result is a clear report, an identifiable next owner, and a record your team can retrieve for follow-up.
Start with the report you need
The Health & Safety page opens the main signature-form chooser. Import an existing Google Form, upload a PDF, or create a starting form from its name. A safety template is a starting point; review the questions, required fields and signer setup for the particular inspection.
For a fictional equipment inspection, collect the equipment identifier, inspection date, observations, result, supporting photos and the inspector's signature. Add the actual questions your team needs rather than an unrelated complete incident-history questionnaire.
Configure the responsible action
- Review the report and required attachments in the editor.
- Decide whether the worker submits the complete report or a supervisor must act next.
- Where a supervisor is needed, configure a workflow signer or approval step with the correct recipient.
- Configure the notification that identifies the report and opens the relevant task.
- Preview on the phone or tablet used for collection, then complete a synthetic report.
Follow up on pending work
Inspect the response status and signature activity to find unfinished tasks. If your workflow uses reminders, configure the relevant schedule and per-step reminder. Check the actual recipient and timing rather than assuming every inspection template arrives with recurring requests or supervisor escalation enabled.
A submitted report and a resolved hazard are different outcomes. Keep the status or staff action required by your process visible. Do not use a signature alone as evidence that remedial work has occurred.
Keep and use the record
Set up Drive saving for the completed documents and verify one saved example. Use response Sheets or reports to filter by equipment, report type or follow-up status. Share historical files through access appropriate to the recipient.
Before rolling out, check attachments, the supervisor's task, the completed PDF, and the correct folder. Mobile-friendly collection does not establish offline support or automatic location capture for every form. The resulting records support your team's process; they do not guarantee an inspection result or certification.
